Cardiology Medication Consultation (CMC) refers to outpatient medication management and optimization for heart-related conditions, typically coordinated by a cardiologist with input from the patient’s primary care provider.

Purpose

Optimize therapy for conditions like coronary artery disease, heart failure, arrhythmias, hypertension, and lipids.

Review current meds for efficacy, safety, interactions, adherence, and tolerability.

Medication reconciliation and optimization during a scheduled outpatient visit.

Telemedicine or in-person visits to assess symptoms, vitals, and lab data (e.g., lipid panels, BNP, kidney function).

Collaboration with primary care for blood pressure, cholesterol, diabetes control, and lifestyle support.

Typical medications involved

Antihypertensives (e.g., ACE inhibitors, ARBs, calcium channel blockers, beta-blockers).
