Cardiology Medication Consultation (CMC) refers to outpatient medication management and optimization for heart-related conditions, typically coordinated by a cardiologist with input from the patient’s primary care provider.

  • Purpose
    • Optimize therapy for conditions like coronary artery disease, heart failure, arrhythmias, hypertension, and lipids.

    • Review current meds for efficacy, safety, interactions, adherence, and tolerability.

  • Common approaches
    • Medication reconciliation and optimization during a scheduled outpatient visit.

    • Telemedicine or in-person visits to assess symptoms, vitals, and lab data (e.g., lipid panels, BNP, kidney function).

    • Collaboration with primary care for blood pressure, cholesterol, diabetes control, and lifestyle support.

  • Typical medications involved
    • Antihypertensives (e.g., ACE inhibitors, ARBs, calcium channel blockers, beta-blockers).

    • Antiplatelets or anticoagulants (e.g., aspirin, P2Y12 inhibitors, warfarin, DOACs) as indicated.

    • Statins and other lipid-lowering therapies.

    • Heart failure therapies (e.g., ACE inhibitors/ARNIs, beta-blockers, MRAs, SGLT2 inhibitors) depending on guideline class.

    • Antiarrhythmics or rhythm-control agents if arrhythmias are present.

    • SGLT2 inhibitors or other agents with proven cardiovascular benefits in appropriate patients.

  • What to prepare
    • Current medication list with doses, frequencies, and any recent changes.

    • Recent lab results and imaging, and a summary of current symptoms or functional status (e.g., dyspnea, chest pain, fatigue).

    • History of side effects or intolerances to prior therapies.

  • What to expect in the visit
    • Review of effectiveness and side effects, assessment of adherence, and any necessary lab monitoring schedules.

    • Adjustments to dosages, additions or discontinuations of medications, and potential drug interactions.

    • Plan for follow-up, including monitoring tests and coordination with other specialists if needed.
