Purpose

Address polypharmacy, psychotropic use, and seizure or behavior-related medications while minimizing side effects and overmedication.

Align treatment with the person’s functional abilities, communication needs, support environment, and long‑term health goals.
What happens

Comprehensive review of all prescriptions, over‑the‑counter drugs, and supplements, with input from caregivers and review of records, labs, and behavior data.

Identification of medication-related problems (e.g., interactions, sedation, behavioral changes) and development of recommendations such as dose changes, deprescribing, or added monitoring.

Creation of a coordinated care plan that integrates medical, behavioral, and environmental strategies, shared back to the primary clinician and support team.
What to bring

Updated medication list, prior evaluations, behavior or seizure logs, and information about mobility, communication, and daily supports.

Questions or concerns from the person and caregivers about side effects, behavior changes, sleep, appetite, pain, or function.
