Geriatric Medication Consultation (GMC)
Geriatric Medication Consultation (GMC) is an outpatient review and optimization of an older adult’s entire medication list to improve safety, reduce side effects, and maintain function.
Purpose

Identify and reduce potentially inappropriate or high-risk medications common in older adults.

Improve adherence, simplify regimens, and align treatment with the patient’s goals and life expectancy.
What happens
-
Comprehensive review of prescriptions, over-the-counter drugs, and supplements, including doses and timing.

Screening using geriatric tools such as the Beers Criteria and deprescribing frameworks to flag risky drugs and interactions.

Development of a tailored medication plan and broader geriatric care plan that addresses medical issues, function, cognition, mood, and social support, in collaboration with the patient and caregivers.
What to bring

Updated list of all medications, vitamins, and herbal products, plus any pill organizers or bottles.

Information on recent falls, confusion, dizziness, hospitalizations, or new symptoms that might be medication-related.
